Family Law - Requests for Information

The Family Law Act provides for the splitting of superannuation interests on marriage breakdown.

Members or their partners can request information about their or their spouse's superannuation account by making a request in writing. The following forms must accompany this request:

  • Form 6 - Declaration by applicant for information about a superannuation interest.
  • Superannuation Information Form - Superannuation Information required for family law matters.

These forms must be completed, signed and dated by the applicant. Blank forms can be provided to members upon request.

A fee of $80.00 (for accumulation funds) and $140.00 (for defined benefit funds, plus the cost of actuarial advice, if required) is payable to cover the processing of the request. These costs are subject to change. The fee is payable by cheque or money order at the time of making the request and should be made payable to 'Group Benefits Pty Ltd'. Should the benefit be split, the withdrawal fee will apply. Refer to the latest Annual Report for details.
