Combined Fund Home Eligibility All employees who are invited by their Employer to join the Fund may apply to become members of the Fund. Spouses of members may also join the Fund. A prospective member must complete an Application for Membership form, which includes a Death Benefit Distribution form. The completed forms should be forwarded to the Employer who will forward them to the Trustee. To be covered for the Fund's death and disablement insurance (if applicable) members may be required to provide evidence of good health or to undergo a medical examination. No insurance cover is provided until we receive a completed Application for Membership form. |
